ACM Medical Laboratory Forensic/DOT Chain of Custody Form
The testing laboratory establishes standard operating procedures for the handling of specimens approved by SAMHSA for DOT and non-DOT testing. "Chain of Custody" procedures ensure that sample identification and integrity are maintained from specimen collection through the testing process.
